We are looking to add qualified case managers to our team.
The Case Manager (QIDP) is responsible for developing, implementing and monitoring each assigned client's active treatment program.
The Case Manager (QIDP) is responsible for the overall management of the assigned work location.
ESSENTIAL DUTIES AND RESPONSIBILITES OF THE CASE MANAGER (QIDP):
Prepare conduct, complete and implement Annual, Special and 30-day Individual Service Plans according to written Policies and Procedures for assigned caseload.
Conduct client ISP's staffing's according to written Policies and Procedures for assigned caseload.
Complete client ISP's after staffing's according to written Policies and Procedures for assigned caseload.
Implement client programs and train all appropriate staff according to written Policies and Procedures for assigned caseload.
Provide effective counseling and case management to client populations Establish and maintain appropriate, current community networks and interactions, in cooperation with program supervisors and staff.
Provide consultation and input to leadership for the program as requested Continue professional growth and development such as cultural competence, social diversity, therapeutic and intervention techniques as related to job duties.
Attend home visits to provide support, assessment and coaching QUALIFICATIONS OF THE CASE MANAGER (QIDP):
Bachelor's Degree in a Human Service Field 1 year of experience working directly with persons with an intellectual disability or other developmental disability.
Highly proficient computer skills and comfortable with Microsoft Office.
Good Oral & Written Communication Skills Highly proficient computer skills and comfortable with Microsoft Office.

Education/
Experience:
Bachelor's Degree in a Human Service Field with at least one year of experience working directly with persons with an intellectual disability or other developmental disability.
Skills Requirements:
Highly proficient computer skills and comfortable with software programs such as Microsoft Office Suite (word, excel, access, power point) or similar software.
Advance knowledge and use of Outlook.
